Location: Cedar Rapids, IA

Hours: Full time, in-person; Monday-Friday 8:00am-4:00pm

Overview of the role

Coordinate client referrals from Law Enforcement Liaisons embedded within law enforcement agencies to provide care coordination, counseling and short-term case management. Conduct standardized assessments and develop client centered case plans to support stabilization goals through creative problem solving, motivational interviewing and trauma informed care. Foster positive relationships with community partners to support clients in navigating various social systems, and connection to individual resources. Facilitate transportation to appointments and other services as requested by the client to promote engagement and decrease barriers to stability. Complete required documentation timely in accordance with program policies and procedures. Attend and participate in team meetings and trainings as required by the program.

Schedule: This position primarily operates during normal business hours Monday-Friday with flexibility in scheduling based on the evolving needs of clients.

What you’ll do

What you need

  • Applicants must possess an A.A. of higher education.
  • Human/social services field preferred.
  • Minimum of 2-year professional experience in behavioral or mental health field required.
  • A combination of education and experience may be considered.
  • High comfort with computers with ability to type quickly and multi-task.
  • Excellent writing and grammar skills.
  • Valid driver’s license and be insurable by the agency auto insurance policy.
  • Pass all background checks completed by Foundation 2 Crisis Services.
  • Submit copy of college degree or transcript as required by licensure for personnel file.
  • Live within 50 miles of Cedar Rapids, IA
  • Be at least 21 years of age.
  • Successfully complete required trainings within required time periods.
  • Experience in crisis services preferred.
  • Education or training in trauma-informed care preferred.
  • Experience in strengths-based practices preferred.
  • Individuals with current certification as a Peer Support Specialist/Peer Recovery Specialist in Iowa are encouraged to apply.
